Hyaluronic acid for skin: benefits, safety and pregnancy
Hyaluronic acid is on a lot of labels, usually with a promise of deep hydration. Here is what the studies measured, and why the size of the molecule keeps coming up.
You may see it written as
- Hyaluronic Acid
- Sodium Hyaluronate
What hyaluronic acid is
Hyaluronic acid (HA) is an important component of the extracellular matrix, the material that supports skin cells. A 2022 review notes that the loss of HA in skin starts at about 25 years of age.1 The review was funded by a cosmetics company, and one of its authors works there.1
In cosmetics it is used for its hygroscopic, rheological and viscoelastic properties.1 In plain terms, it holds water and gives a product body.
What it does for hydration and wrinkles
The most useful trial we found tested 0.1% HA creams of five different molecular weights, from 50 to 2,000 kDa, on 76 women aged 30 to 60 with wrinkles around the eyes. They applied one of the creams twice a day for 60 days, and a plain vehicle cream around the other eye. All the HA creams improved skin hydration and elasticity compared with the plain cream. Wrinkle depth improved significantly only with the 130 and 50 kDa creams.2
The authors suggest the smaller molecules may have worked better because they penetrate better.2 That is their interpretation. It is one trial, around the eyes, over two months, so it cannot tell you which size to buy.
Reviews are more upbeat. A 2022 review concludes that numerous studies support HA-based cosmetics for improving hydration and rejuvenation.1 A 2018 review reports anti-wrinkle and anti-ageing effects, but its evidence includes injected fillers, which are a different thing from a cream.3
Side effects and sensitivity
The 2022 review reports that several clinical studies indicate HA is well tolerated.1
Reactions to HA creams do happen, but the cause is not always HA. In one reported case of allergic contact dermatitis from a hyaluronic acid cream, the culprit was a preservative in the cream, not the hyaluronic acid.4 If a product irritates your skin, stop using it.
Using hyaluronic acid with other products
We did not find a source saying hyaluronic acid must be kept apart from another active. The studies we found tested it alone, against a plain cream, so they cannot tell you how it behaves layered with other products.
Add one new product at a time. If your skin reacts, you will know which one did it.
Hyaluronic acid in pregnancy and breastfeeding
Hyaluronic Acid, Sodium Hyaluronate
We found no pregnancy- or lactation-specific evidence for this ingredient.
No pregnancy-specific evidence is not the same as safe, or as unsafe.
Evidence on cosmetic products in pregnancy is limited overall, so ask your doctor or midwife about the specific product you use.5
What you will see on the label
Look for Hyaluronic Acid and Sodium Hyaluronate. Snifff has an entry for each and shows both as Risk-free. For the rest of the formula, see what the score means and how to read the list.
The trial above used 0.1% creams,2 so compare that with what your product says before reading across.

A FEW MORE ANSWERS
Common questions about hyaluronic acid
Does hyaluronic acid really hydrate skin?
In a trial of 76 women, all of the 0.1% HA creams improved skin hydration and elasticity compared with a plain cream.2 Reviews report the same direction.1,3
Does hyaluronic acid reduce wrinkles?
In that trial, wrinkle depth improved significantly only with the 50 and 130 kDa creams, after 60 days, around the eyes.2 It is one study, so treat it as a hint, not a promise.
Which molecular weight of hyaluronic acid is best?
The trial found wrinkle depth improved with the two smaller sizes it tested and not the three larger ones.2 One trial cannot pick a best size.
Can hyaluronic acid cause an allergic reaction?
A 2022 review reports that HA is well tolerated,1 and in one reported case of a reaction to an HA cream, a preservative was to blame.4 Stop using a product that irritates you.
Is hyaluronic acid safe in pregnancy?
Snifff found no pregnancy-specific evidence for Hyaluronic acid. Ask your doctor or midwife about the product you use.
